LCSW, LMSW, CSW, LPC, PLPC SPECIFIC RESPONSIBILITIES AND DUTIES 1.
Completes psychosocial assessments on all newly assigned clients and within time frames as mandated by hospital policy and regulatory standards.
Provides and documents clinical individual, group, and family therapy (as ordered), in addition to Discharge Planning.
Assesses social and emotional factors to facilitate the patient's and family's compliance with treatment recommendations to optimize the patient's level of functioning.
Coordinates members of the multidisciplinary clinical team (nursing, physician, other counselors, patient, and the family) to address the biopsychosocial needs of the patient.
Supervises the development, implementation, assessment and revision of the Multidisciplinary Treatment Plan within required time frames.
Maintains a list of, and contact with, community resources for aftercare, placement, and support services to assist the patient and his/her caregivers after discharge from Beacon.
Keeps abreast of behavioral health trends and knowledge in social services.
Actively participates in:
a.
Identifying opportunities for improvement to be included in the Quality 14.
Immediately reports alleged, suspected, or witnessed abuse/neglect and any unusual behaviors or occurrences to the Charge Nurse and the Director of Social Services, as well as to the Director of Nursing, Administrator, or Administrator on Call.
Provides care to patients in accordance with:
a.
Prescribers' Orders; b.
Hospital Policies & Procedures; c.
Scope of Practice; d.
State & Federal Regulations; e.
State & Federal Statutes; and f.
Industry Standards of Care.
Communicates significant changes in a patient's condition to the Charge Nurse.
Communicates significant changes in a patient's condition and communication from caregivers to the treating practitioner.
Performs other duties and special projects as required within Cypress Grove Behavioral facilities.
